Perspectives on Lifestyle and Treatment in Inherited Heart Rhythm Disorders

Dear Patient or Family Member,

I am taking this opportunity to provide you with some information about an online survey you may be eligible to take part in.

The purpose of this survey is to determine the lifestyle changes that patients with an inherited heart rhythm disorder experience in terms of physical activity and implantable cardioverter defibrillator (ICD) intervention since their diagnosis. Patients diagnosed with an inherited heart rhythm disorder and their caregivers are eligible to participate.

The data from this survey will be used to support the aims of a Heart and Stroke Grant application by identifying which aspects of lifestyle and treatment in inherited heart rhythm disorders need to be prioritized for research.

Please note that no part of the survey will have any information that could be used to identify you. There is no compensation for responding nor is there any risk. This survey will take less than two minutes to complete.
